Safety, Contraindications & Traditional Use
Medication interactions (internal use). St. John's Wort speeds up the way the body processes many drugs by inducing the CYP3A4 enzyme and P-glycoprotein. Taken internally, it can lower the effectiveness of a wide range of medications, including hormonal contraceptives (with a documented risk of contraceptive failure), warfarin and other anticoagulants, cyclosporine and tacrolimus, digoxin, certain HIV antiretrovirals, some chemotherapy agents, and statins, among others. This effect can persist for weeks after you stop taking it.
Serotonin syndrome. Combining St. John's Wort with serotonergic medications, including SSRIs, SNRIs, MAOIs, triptans, tramadol, or buspirone, can lead to serotonin syndrome. Do not combine it with antidepressants or other serotonergic drugs without medical supervision.
Photosensitivity (internal and topical use). The constituent hypericin can increase the skin's sensitivity to sunlight. Whether used internally or applied topically, limit strong sun and UV exposure, particularly if you are fair-skinned.
Pregnancy, nursing, and children. St. John's Wort is not recommended during pregnancy or breastfeeding, or for young children, except under the guidance of a qualified practitioner.
Topical preparations. Always patch test an infused oil or salve on a small area first, discontinue use if irritation develops, and avoid direct sun on treated skin.
